Circulatory Dynamics in the Presence of Carotid-jugular Fistula: Effects of Repair after Twelve Years' Patency.

چکیده
THE collateral circulation that develops invariably in the vicinity of a systemic arteriovenous fistula has impressed generations of surgeons who have treated these lesions. Investigators in various surgical modalities have sought to apply this principle to enhance the blood supply of specific regional tissue. The most successful application to date has been the creation of femoral arteriovenous fistulas in children with shortened atrophic limbs, by the technic of Janes and Jennings.' Chatterjee and Warren 2 are investigating the effect of internal mammary arteriovenous fistulas on the coronary circulation. The concept of arterialization of the cerebral venous system to augment cerebral blood supply was introduced by Sciaroni3 in 1948. Beck and associates 4 modified Sciaroni's procedure of cross anastomosis and created sideto-side fistulas between the right internal jugular vein and the common carotid artery, with central ligation of the vein. The procedure was directed at the entities of organic brain disease, especially juvenile epilepsy and mental retardation. On the thesis that such lesions were characterized by gliosis of brain cells, it was anticipated that additional blood flow might penetrate the scar tissue and reactivate such cells. From 1949 to 1953, 363 patients in the United States and Europe were reported in whom carotid-jugular fistulas were produced.5-16 On the basis of this accumulated experience, the procedure was deemed without permanent value, and no further cases have been reported. An adult case of congenital external carotid-jugular arteriovenous fistula with reversible circulatory insufficiency in the cerebellum and lower brain stem.
A 27-year-old man with congenital external carotid-jugular arteriovenous fistula presented with a diminished level of consciousness and an ataxic gait. Axial fluid-attenuated inversion-recovery (FLAIR) MR imaging revealed venous congestion, a dilated right jugular vein, and an area of high signal intensity in the brain stem and cerebellum. Angiography showed a dilated right external carotid art...

Introduction: Penetrating trauma to anterior neck can induce cerebral ischemia due to carotid artery injury. Brain ischemia also can present after surgical carotid repairs. Early diagnosis and suitable treatment modality prevent from permanent neurologic deficit post operatively. Case Report: A 30 years old man with stab wound to zone two left side of neck underwent exploration and penrose i...
متن کاملComparison of Six-Month Patency Rate in Ulnar-Basilic Fistula and Radio-Cephalic Fistula for Hemodialysis Access
Background and Objectives: There are different types of vascular access. We compared the patency rate for six months in ulnar-basilic fistula and Radio-Cephalic fistula for vascular access in hemodialysis patients. Methods: The study was performed on 60 patients of 50.6±13.5 years of age with end-stage renal failure undergoing hemodialysis from 2012-2014 in Imam Ali hospital in Zahedan. Pat...

متن کاملA Lumped Parameter Method to Calculate the Effect of Internal Carotid Artery Occlusion on Anterior Cerebral Artery Pressure Waveform
Background and Objective: Numerical modeling of biological structures would be very helpful tool to analyze hundreds of human body phenomena and also diseases diagnosis. One physiologic phenomenon is blood circulatory system and heart hemodynamic performance that can be simulated by utilizing lumped method. In this study, we can predict hemodynamic behavior of one artery of circulatory system (...
